Two passport-size photos with a plain white background (one attached to the form, one submitted separately)

Confirmed return airline ticket

Yellow Fever vaccination certificate (including Polio record where required)

Original signed and stamped introductory letter from employer, school, or company stating purpose of travel

Kenyan passport valid for at least six months (old passports must also be provided if available)

Passport must be submitted without protective covers, pins, or staples

Clear copy of passport bio-data page attached to application

Non-Kenyan nationals must provide a copy of their residence permit

Six months’ bank statement and an introduction letter from the bank
